For Community Referral Organizations

One call, and the family is taken care of.

Prime Infusions is a network of infusion centers across the New York metro area, built around the needs of our kehillos. When you have a family who needs infusion therapy, you shouldn’t have to figure out whether we can help, where to send them, or what happens next. This page answers all three.

02How to Refer

How to refer a family

One call is all it takes. You give us the patient and the treating physician; we take it from there.

  1. Send it over

Call, fax, or email us the patient’s name, the treating physician, and the insurance information if you have it. If you only have a name and a phone number, send that — we’ll chase the rest.

  2. We handle the back end

We contact the physician’s office for orders, verify benefits, and pursue prior authorization. No paperwork lands on your desk.

  3. The family gets scheduled

We book them at the center closest to home and confirm directly with the family. We’ll update you so you can close the loop.

Helpful to have

  • Patient name and phone number
  • Treating physician and practice
  • The therapy or medication ordered
  • Insurance carrier and member ID
  • Preferred location or neighborhood

Not needed

  • Prior authorization — we do that
  • Chart notes or lab work — we request them from the physician
  • A completed referral form — a phone call is enough

Four ways to send it

For a medical emergency, call 911.

A one-page cover sheet with our fax number and blank fields for everything above. Print it, fill it in, fax it back.

Over 42 therapies and growing. If you don’t see what you’re looking for, call us — we usually have it or can get it.

Cancer-related and supportive infusions

The searchable list above is our standing referral formulary — the therapies that each have their own referral form. It is not the whole picture for cancer care. We also administer:

  • Prostate cancer — hormonal therapy support · Lupron referral form
  • Breast cancer — supportive infusions
  • Multiple myeloma
  • Hemophilia A & B, and sickle cell disease
  • Chemotherapy-related anemia — Procrit, on the list above

If you’re placing a family for cancer care, call intake at 718-443-4000 with the specific order in hand. We’ll tell you whether we can take it before anyone commits the family to an appointment.

05Built for Our Community

Built for our community

Clinical quality is the baseline. What families remember is whether they felt comfortable.

Private treatment rooms

Every infusion happens in its own room with a comfortable chair — not an open bay. Families can bring someone along, and nobody sits on display.

Heimishe refreshments

Complimentary snacks families will actually eat. Infusions can run several hours; nobody should have to pack a bag or go hungry.

Sensitive to community needs

Our staff understand what a frum family needs from a medical setting, and treat every referral with discretion.

A calm, modern facility

Current equipment, an experienced clinical team, and free WiFi and entertainment at every chair.

06Insurance

Insurance and authorization

We accept most major insurances, including Fidelis. Our team handles benefit verification and prior authorization directly with the carrier and the prescribing physician — the family doesn’t chase it, and neither do you.

If a family’s coverage is uncertain, send the referral anyway. We’ll verify benefits and tell you where things stand before anyone commits to an appointment.
